  This search is used to find home and community care services, which cater for someone living in a particular suburb or town of South Australia.

To search services, you make selections in any or all of the following criteria. The more criteria you specify the more precise and relevant your search results will be.

Service

Service categories are arranged in a scroll box. There are broad service categories in uppercase and sub-categories in title case. Sub-categories are arranged under the broad categories. For definitions of the service categories see the glossary.

You can navigate in this scroll box in one of three ways:

  • Use your mouse to drag the scroll bar up or down or click on the up or down arrows at either end of the scroll bar
  • For those who prefer the keyboard, tab to the scroll box with the Tab key and either use the arrow keys or the Page Up and Page Down keys on the keyboard to navigate up or down the list OR
  • Highlight a category by clicking on it and enter a letter, such as 'H'. This will highlight the first broad category beginning with that letter. Enter the same letter again to go to the next broad category beginning with that letter etc.
You can select a broad category or a sub-category. Selecting a broad category will search for all of the sub-categories arranged under that broad category.

Click on a category with your mouse to select it. Selected services will be highlighted blue. To select multiple service categories hold down the CTRL key while you make your selections with the mouse.

Suburb/Town

All suburbs and towns in South Australia are listed in the drop-down box in alphabetical order. By default 'Any suburbs/towns' is selected. If you select a suburb, the search results will include only those requested services that cater for that suburb. If you select 'Any suburbs/towns' for suburb, search results will include all requested services anywhere in the state of South Australia.

To navigate through the list to find the suburb/town, tab to the field and type the first few letters of the suburb. If you make a mistake press the Esc key on your keyboard. For example, if you are looking for services for someone who lives in Murray Bridge, select Murray Bridge from the list.

Council

All council areas in South Australia are listed in the drop-down box in alphabetical order (Some have been divided into former council areas because many services are based on these boundaries). By default 'Any councils' is selected. If you select a council area the search results will include only those requested services that cater for that council or part thereof. Note you cannot select a suburb AND a council area.

Fee Type

There are two fee types: Full fee for service and Subsidised/No fee. 'Any fee types' is selected by default. To narrow the search make a selection from the drop-down list. For example, select Subsidised/No fee if you are searching for low cost or free services.

Cultural Focus

By selecting categories in this field you can narrow your search to those services that have a particular cultural focus.

The options are:
  • Aboriginal & Torres Strait Islander - select to look for services that cater specifically for indigenous clients
  • Ethnic - select to look for services that cater specifically for clients of culturally diverse background
  • General - select to find services that cater for the general community
You can select more than one option.

Client Focus

Some services focus on or are restricted to particular target groups. This field enables you to narrow your search to services which cater for a particular target group.

The options are:
  • Aged - aged 60+ and frail aged
  • Disability - younger people (under 65) who have a permanent and severe/profound physical, intellectual or sensory disability or brain injury
  • Carers - unpaid carers of the aged or people with a disability
  • Veteran - war veterans
  • Dementia - people with memory loss conditions such as Alzheimer's Disease
  • Mental Health - people with a mental health disability such as depression, agoraphobia and schitzophrenia
  • Post-acute - people who have been recently discharged from an acute episode in hospital
  • General/Other - the general community or other target groups
You can select more than one option.

Glossary

The following is a glossary of the service headings used on Commonwealth CarelinkSA and their definitions. They are arranged with broad categories first followed by sub-categories.

ACCOMMODATION - RESIDENTIAL AGED CARE FACILITY

Services which provide accommodation, personal care, health care, treatment and supervision at a special-purpose facility. The purpose of such activities may be to provide ongoing support or long-term nursing care as well as accommodation.
  • Residential Accommodation - High Level Care - Australian Government-subsidised high level aged care in a residential facility. It also includes long stay high care care beds funded by the State Governement in country hospitals.
  • Residential Accommodation - Low Level Care - Australian Government-subsidised low level aged care in a residential facility. It also includes long stay low care beds funded by the State Governement in country hospitals.

ACCOMMODATION - SUPPORTED (DOMESTIC SCALE)

Services that provide care and supported accommodation under the care of one or more adults, which may be for the purposes of transition to independent living.
  • Group Home(s) - accommodation for people with a disability operated as a single household. It may be individual units or congregate living and usually provides care/support
  • Information and Support (accommodation) - information and/or support to find suitable accommodation and support services

ACCOMMODATION - SUPPORTED (FACILITY)

  • Short Term Accommodation (Carers / Patients) - mostly accommodation for people who need to travel to Adelaide or a regional centre for medical treatment and / or their families and carers. It also includes schemes providing financial assistance to cover for this type of accommodation. It does not include general holiday accommodation.

ALLIED HEALTH CARE / REHABILITATION

Also known as paramedical care. Refers to allied health care services provided by a qualified professional.
  • Alternative Therapy - traditional medicine disciplines such as naturopathy, homeopathy, massage therapy, acupuncture, chinese herbal medicine etc.
  • Therapy Service - a Commonwealth-subsidised service which provides a range of allied health services
  • Dental
  • Diet / Nutrition
  • Hydrotherapy
  • Occupational Therapy
  • Optometry - In Home
  • Physiotherapy
  • Podiatry
  • Podiatry - In Home
  • Rehabilitation - therapy, which can be multidisciplinary, which has the aim of restoring health. Does not include gambling, alcohol or drug rehabilitation
  • Social Work
  • Speech Pathology
  • Therapy Service - a service subsidised under the Australian Government's Day Therapy Centres scheme providing a range of allied health services for older people under a single roof and with a multidisciplinary approach.

ASSESSMENT SERVICES

All assessments and re-assessment activities, which are directly attributable to individual clients.
  • Assessment - Aged Care Assessment Team (ACAT) - assessment by an Aged Care Assessment Team to determine a person's eligibility for respite and permanent residence in Commonwealth-funded aged care facilities (high and low), for a Community Aged Care Package (CACP), an Extended Care at Home (EACH) package, an Extended Care at Home (EACH) package or for the Transition Care Program.
  • Assessment - Community Care - assessment to receive services to help people remain independent in their own home. It is mostly used for HACC-funded services and dfined according to the Home and Community Care (HACC) Program guidelines.
  • Assessment - Disability - assessment to receive disability services, usually provided, funded or brokered by State Government organisations.

CARE COORDINATION, CASE PLANNING, REVIEW

Activities related to the coordination, planning and delivery of services which are directly attributable to an individual client. Relates to less complex tasks usually restricted to a single agency.
  • Care Coordination/Planning

CASE MANAGEMENT

Assistance received by a client from an agency who is responsible for the coordinated delivery of services from more than one agency to a client with complex care needs. Usually involves a formal agreement.
  • Case Management

CENTRE-BASED DAY CARE

Structured group activities designed to develop, maintain or support the capacity for independent living and social interaction, which are conducted in a centre-based setting. This also includes outings and day trips conducted by a day care centre.
  • Centre Based Day Care - Aged
  • Centre Based Day Care - Dementia - special program of activities or support provided for people with dementia
  • Centre Based Day Care - Disability

COUNSELLING/SUPPORT, INFORMATION AND ADVOCACY

Assistance with understanding and managing situations, behaviours and relationships associated with the person’s need for care and/or the caring role, including advocacy and the provision of advice, information and training. It includes dementia support and counselling, on a one-to-one basis.
  • Advocacy - Individual - education, support or representation to assist individual consumers to attain their rights. It does not include the mere arrangement or coordination of services on a client's behalf; nor does it include systemic advocacy and lobbying.
  • Carer Support - support, counselling or advice for carers to assist them in their caring role. Does not include practical support or respite. Examples include carer support group, telephone support, training, counselling for carers
  • Counselling - general counselling or counselling in relation to a disability. Relationship counselling and financial counselling are not included
  • Dementia Support - support, counselling or advice for someone with dementia or someone caring for someone with dementia. Does not include practical support or respite. Examples include carer support group, telephone support, training, counselling for carers
  • Information and Referral - provision of an information & referral service, as a primary/exclusive role. This involves developing resources and keeping them up-to-date

DISABILITY SERVICES - OTHER

Services for a person with a disability.
  • Day Options / Post School Options - structured program of centre- and / or community-based recreational and educational activities
  • Living Skills Programs - programs aimed at providing knowledge and experience needed to participate in the community. These competences may include personal grooming, cooking, eating and drinking, housework, use of money, social skills, use of public transport, etc.

DOMESTIC ASSISTANCE

Assistance with domestic chores.
  • Cleaning
  • Ironing/Laundry
  • Shopping/Banking - client can be accompanied or it is done on their behalf. Payment of bills is also included

FORMAL LINEN SERVICE

The provision and laundering of linen, usually by a separate laundry facility or hospital.

HEARING SERVICES

Hearing services provided by an accredited hearing service provider.
  • Hearing - Assessment and Testing

  • Hearing - Support and Education

HOME MAINTENANCE

Assistance with the maintenance and repair of the person’s home, garden or yard to keep their home in a safe and habitable condition.
  • Gardening
  • Home Maintenance - minor repairs e.g. gutter cleaning, replacing tap washers/light globes etc
  • Rubbish Removal

HOME MODIFICATION

Assistance with modifications or renovations to the person’s home to help them cope with a disabling condition.
  • Home Modification - includes larger modifications, such as bathrooms, kitchens, etc. to adapt for a disability, and minor modification such as the fitting of safety rails, ramps, etc, for the health, safety and continued independence of client. Comments field is used to clarified specific types of service provided
  • Safety/Security Assessments - Safety Assessment - identification of home modification needs for the health & safety of the client, usually performed by an occupational therapist. Security Assessment - identification of home security needs

MEALS

Refers to the receipt of a prepared and delivered meal.
  • Meals - Frozen
  • Meals Delivered - Hot

MEDICAL / HEALTH SERVICE

Service that provides health treatment, advice and support.
  • Continence - advice and continence nursing
  • Diabetes Education
  • Exercise Groups
  • Falls Prevention
  • Health Education and Self-management - health education and self-management programs - e.g. for stroke, Parkinson's, diabetes etc.
  • Mental Health Service - assessment and treatment for people with a mental illness such as depression, schizophrenia etc. This category does not include other services (e.g. respite) for people who suffer from a mental illness. In such cases search for the relevant service category then select 'Mental Health' as target group (under 'Client Focus').

NURSING CARE

Professional care from a registered or enrolled nurse.
  • Nursing - Clinic Based - e.g. Regional Health Service or Royal District Nursing Service (RDNS) clinic
  • Nursing - In Home

OTHER FOOD SERVICES

Assistance with the preparation and cooking of meals in the client’s home and the provision of advice on nutrition, storage or food preparation.
  • Food - Delivery - delivery of groceries
  • Meals - Assistance with Preparation

PACKAGES OF CARE

Individual and coordinated packages of community care designed to meet the daily needs of frail older people with complex care needs.
  • Community Aged Care Packages (CACP) - packages of care (low care) provided in the home and subsidised by the Australian Government
  • Extended Aged Care at Home (EACH) - packages of care (high care) provided in the home and subsidised by the Australian Government
  • Extended Aged Care at Home Dementia (EACHD) - packages of care (high care) provided in the home and subsidised by the Australian Government for people with complex needs associated with dementia, especially challenging behaviour.
  • Hospital avoidance / discharge services - packages of support services provided in the home (personal care, domestic assistance, assistance with meal preparation, etc.) aimed at avoiding an unnecessary hospital admission or to reduce length of stay.
  • Packages of Care - a package of care services provided to a client in the home by one agency. Not government subsidised

PALLIATIVE CARE

Service that provides health treatment, supervision and support, which is aimed at maintaining or improving the comfort of a patient who is, or would otherwise be, in pain or distress.
  • Palliative Care - In Home
  • Palliative Care - In Hospice/Hospital

PERSONAL CARE

Assistance with daily self-care tasks such as eating, bathing, toileting.
  • Hairdressing - in-home or centre-based hairdressing
  • Medication Monitoring - home visiting or via telephone to check clients are taking their prescribed medications and in appropriate amounts
  • Personal Care - assistance with self-care tasks such as showering, dressing, eating, toileting or grooming

PROVISION OF GOODS AND EQUIPMENT

Loaning or purchasing of goods and equipment to assist the person to cope with a disabling condition and/or maintain their independence.
  • Equipment - Advisory Service

  • Equipment - Loan/Subsidised
  • Equipment - Modification
  • Equipment - Purchase
  • Equipment - Second Hand
  • Equipment - Personal Alarms - usually monitored telephone-based emergency call system which is activated by a pendant worn on the person. Sale, installation and/or hire of personal alarms

RESPITE CARE

Assistance received by a carer from a substitute carer who provides supervision and assistance to their care recipient in their absence.
  • Day Respite -Out of Home - respite provided out of the home by a substitute carer. Also includes occasional day respite provided by a facility/centre
  • Overnight Respite - Out of Home - overnight respite in a facility such as a 'respite cottage'.
  • Re spite - Emergency
  • Respite - Holiday - supporting frail aged, people with a disability and their carers to have a holiday
  • Respite - Host Family - respite provided in a host family's home. Not an outing
  • Respite - In Home - respite provided by a substitute carer in the care recipient's home
  • Respite - Information and Advice - guidance on where to find respite care that is suitable for carers and care recipients; help with planning for respite.
  • Respite - Residential Aged Care Facility - provided in a Australian Government funded low and high care residential aged care facilities. Care recipient must have been assessed as eligible by an Aged Care Assessment Team (ACAT)

SOCIAL SUPPORT

Assistance provided by a companion (paid worker or volunteer) either within the home or while accessing community services or facilities, which is primarily directed toward meeting the person’s need for social contact and/or accompaniment in order to participate in community life.
  • Assistance with Social Integration - assisting someone to access local support/social groups
  • Friendly Visiting - usually a volunteer matched with a client who visits the client in their own home on a regular basis
  • Outings - individual and group where the outings are the primary activity and not part of a broader program of supported recreation or centre-based day care
  • Supported Recreation - recreation where support and/or transport is provided to enable/assist elderly people and people with a disability to participate
  • Telephone-Based Monitoring Service - daily monitoring of client via telephone
  • Telephone Support - social support provided over the telephone

TRANSPORT

Assistance with transportation either directly (eg a ride in a vehicle provided by or driven by an agency worker or volunteer) or indirectly (eg taxi vouchers or subsidies).
  • Transport - Community Van/Bus
  • Transport - Disability Parking - disability parking permits provided by the Government
  • Transport - Personal - personal transport in a vehicle provided by or driven by an agency worker or volunteer
  • Transport - Taxi and Other Subsidies
